If you have the Acrobat Reader (most newer browsers come with it):

1 -- Click on the graphic to the left and the form will download to your computer.

2 -- Most web browsers will then open the file in the Reader program but if yours doesn't, open the Reader and use it to open the downloaded file (named grform.pdf)

3 -- Then, (this is VERY important), put your mouse right over the document and click it once. This screen actually combines several "frames" (different pages) and your browser must know which frame you want to print out. After doing that, just print it out using the Browser's "Print" function.

If you don't have it: 1 -- Download from this Adobe's Acrobat Download page. It's free and it's worth the time downloading because this format is becoming very popular on the Internet.

2 -- Once the Reader is downloaded to your computer install it by double clicking on its icon or following the installation instructions on Adobe's web page

2 -- Come back to this web page and click on the form graphic to the left

3 -- Then just follow the same procedure as #3 above.
