Dear Member,

As Trustees, we are pleased to provide you with this updated booklet describing the benefits offered by Social Service Employees Union Local 371 Annuity Fund Plan.

The Trustees have appointed John D. Brown as Plan Administrator and Helen Wilson as Associate Administrator of the Plan. On behalf of each Member employed on June 1, 1999, an Individual Account was established to which the City of New York made a onetime contribution, based upon the number of days worked, in accordance with the provisions set forth in the April 1, 1995 to March31, 1999 Collective Bargaining Agreement between the Social Service Employees Union, Local 371 and the City of New York (the “Original Agreement”).

As the result of collective bargaining, each Member employed on or after June 30, 2002 has an Individual Account maintained on his or her behalf by the Plan. The City of New York makes regular contributions to the Plan, based on the number of days worked, in accordance with the provisions set forth in the April 1, 2000 to June 30, 2002 Collective Bargaining Agreement between the Social Service Employees Union, Local 371 and the City of New York (the “2000-2002 Agreement”).

The contributions are added to the existing accounts of Members employed on June 1, 1999, and to new accounts established for new members. Under the provisions set forth in the Collective Bargaining Agreement between the Social Service Employees Union, Local 371 and the City of New York (the 2005-2008 Agreement), which covers the period July 1, 2005 through March 2, 2008, each Member actively employed in covered titles on or after March 2, 2008 will receive an increased level of contributions to his or her Individual Account maintained under the Plan, based on the number of days worked.

In the following pages of this booklet you will find the answers to specific questions that members frequently ask. It is suggested you become familiar with the contents of this booklet and keep it in a safe place for handy reference.
